Glaucoma is a progressive condition that damages the optic nerve, and without treatment it can lead to permanent vision loss. The most established surgical treatment is trabeculectomy, which creates a new drainage pathway to lower intraocular pressure. It has the longest track record and remains the gold standard for moderate to advanced glaucoma. Tube shunt surgery (glaucoma drainage device) is an alternative for eyes where trabeculectomy has failed or is unlikely to succeed.
For milder or earlier-stage glaucoma, MIGS (minimally invasive glaucoma surgery) offers a lower-risk option that can be performed on its own or combined with cataract surgery. Selective laser trabeculoplasty (SLT) is a quick, outpatient laser procedure that improves fluid drainage and can delay or reduce the need for drops.
All glaucoma patients receive comprehensive assessment including visual field testing, OCT nerve fibre analysis, and pachymetry before any treatment decision is made. Surgery is followed by a structured schedule of pressure checks and medication adjustments to achieve optimal long-term control.
